How to register your 3X4 DNA Sample Kit
For Customers: Registering your 3X4 Kit
When you order a 3X4 Genetics test, you'll receive a DNA sample kit to collect your sample with a quick, painless cheek swab.
Before returning your sample, you must register the unique kit code printed on your sample vial. This links your sample to your account. Unregistered samples may be discarded after arriving at the lab.

How to register your kit
If you were shipped a 3X4 kit after August 13th, 2026, your kit insert will instruct you to scan the QR code on the bottom of the kit, or visit 3X4genetics.com/register-kit. Alternatively, select the Create Account link in your Kit Instructions email.
⚠️ Kits shipped prior to August 13th, 2026 cannot register using the above link. Refer to the the instructions inside your kit, and as detailed below.
Step 1: Enter Serial Number
Enter the serial number printed on the outside of the 3X4 kit tube, or scan the QR code. On-screen guidance shows exactly where to find it, with example formats.
- New accounts must verify their email address.
- A person can only register their own kit.
- Only one kit can be registered per profile; a second kit needs a new profile.
Step 2: Claim Kit
If your kit was ordered or supplied to you by your practitioner, you will see your practitioner's name on the claim kit screen, and will be prompted to create an account with your own email address.
A kit can only be registered to one email address and account.
You must provide the email address the kit will be registered to. Your results account will be tied to this address.
Step 3: Enter Verification Code
A 6-digit code is sent to your email. Entering it proves you control the address before registration continues.
Step 4: Create Account
With the email verified, you create your account, review and accept the Terms of Service and Privacy Policy, set a password, and confirm consent before continuing.
Step 5: Choose Recipient
Choose who will use the kit: Myself or Someone Else.
⚠️ The account holder can only register their own kit
If the sample will be collected by anyone other than the account holder, you must choose Someone Else. You can only register your own kit through your own account.
Step 5B: Transfer Kit to Someone Else
Choosing Someone Else lets you transfer the kit after entering the recipient's email address. The recipient receives a secure link and completes registration under their own account.
Step 6: Personal Information
Enter your name, phone number, date of birth, sex assigned at birth, and biological ancestry information. Sharing your biological ancestry is optional, but will allow us to tailor future versions of your report.
Step 7: Enter Kit Code
Enter the kit code from the DNA sample collection vial packed inside the kit (format KUSA000000).
The kit code is how your sample will be identified at the lab
The lab only identifies your sample using the kit code on the DNA sample vial, protecting your privacy. The serial number on the outside of the tube is no longer required.
Step 8: Confirm Linked Practitioner (if applicable)
If your kit was ordered or supplied to you by your practitioner, you will see your practitioner's name pre-filled and fixed. By continuing, you consent to share your genetic test results with your practitioner.

If the named practitioner is not what you expected, stop and contact 3X4 support.
Step 9: Track Sample Return
Capture the return tracking number printed on the shipping bag label, if one was supplied (optional).
⚠️ Biological sample shipping regulations apply
Samples ship as Exempt Human Specimens. Make sure 3X4's packaging instructions are followed.

Finally, you are taken through the next steps: collecting your DNA sample and shipping your sample to the lab for processing. Ensure that you carefully follow the instructions provided.

Following 3X4's sample collection instructions is essential to help prevent sample failure at the lab.
Registration instructions for kits shipped before Aug 13th, 2026
Step 1: Create Profile
Using the QR code or link on the kit insert provided inside the kit, you first create a 3X4 profile, or log into your existing 3X4 account.
- If creating an account, you are required to verify your email address.
- The person creating their profile can only register their own kit.
- Only one kit can be registered per profile.
Step 2: Complete Profile
On the following screen, enter your name, contact number, and sex assigned at birth.
Step 3: Enter Kit Code
Enter and confirm your kit code.
Step 4: Link Practitioner
Enter your practitioner's unique 3X4 practitioner code. You can find this in your Kit Instructions email, or ask your practitioner to provide it to you. This code is available on the 3X4 practitioner portal, on the profile page.
Step 5: Link the Return Tracking Number
For kits supplied with a shipping bag and label, you can capture the tracking number on 3X4's systems (optional).
USPS shipments can only be returned at USPS branches, and FedEx at FedEx branches.
