How to order using HSA/FSA

If your HSA/FSA provider supports direct payments, you can use your HSA/FSA debit card at checkout like a regular credit card. If direct payment isn't supported, pay with a standard credit card and submit your itemized receipt for reimbursement.

Can I claim retroactively?

• HSA - Yes, as long as your HSA account was active at the time of purchase.
• FSA - Yes, but only if the purchase was made within your current plan year.

What if my provider purchased the test on my behalf?

Ask your provider for an itemized receipt and submit it directly to your HSA/FSA provider for reimbursement.

How do I request an itemized receipt from 3X4 Genetics?
